The Student Evaluation Form is a printable tool that allows instructors to record skill evaluations by hand instead of entering them directly in the Staff Portal. Unlike Class Evaluation Forms, which are organized by class, Student Evaluation Forms are organized by individual student and include all skills tied to that student’s enrollments in one place.

The Student Evaluation Form functions as a paper-based alternative for tracking skill progression. It is especially useful in situations where instructors prefer to record evaluations offline and enter them into the System at a later time.

- Grouped by class.
- List each student in the class separately.
Because Student Evaluation Forms consolidate evaluations by student, they are often used for focused assessments, private instruction, or progress reviews.
Accessing the Feature
Student Evaluation Forms are generated from the Students page in the Office Portal.
To access this feature:
- Navigate to Students from the main navigation menu.
- Locate the student or students you want to evaluate.
Step-by-Step Instructions
Follow these steps to generate a Student Evaluation Form:
- Go to the Students page.
- Select the checkbox next to one or more students you want to evaluate.
- Select the Reports icon in the task bar to open the reports menu.
- Select "Student Evaluation Forms."
- Review the form filter screen and adjust the available options as needed:
- Levels.
- Events.
- Skills.
- If no selections are made, all available levels, events, and skills are included by default.
- Select PDF to generate and print the form.
The generated PDF can be printed and used by instructors to record evaluations manually.

What happens if I do not select any levels, events, or skills on the filter screen?
- The System automatically includes all available levels, events, and skills associated with the selected student(s).

Can Student Evaluation Forms be completed directly in iClassPro?
- No. Student Evaluation Forms are designed as printable documents. Skill evaluations recorded on the form must be entered manually into the System later if digital tracking is required.

How is this different from the Student Evaluation Report?
- The Student Evaluation Form is a printable worksheet for recording evaluations, while the Student Evaluation Report displays evaluation data that has already been entered into the System.
