What is the Class Evaluation Form?
The Class Evaluation Form is a printed alternative to recording skill evaluations via the Staff Portal. This report contains student and skill names so that instructors may record written evaluations that can later be recorded in the system. This report will also display the students’ previous evaluation details for reference.
How Do I Generate a Class Evaluation Form?
Step 1
To print a class evaluation form, go to the CLASSES page and select the class(es) for which you are needing to record an evaluation.
Step 2
Click the reports icon in the task bar to open the reports menu. Select "Class Evaluation Forms."
Step 3
This will bring up the form summary window which shows the of classes selected and as of which date the report is being generated for.
By default, the "as of" date will be set to the current date. For accuracy, this date should be the date that the evaluations will be performed on to include the correct students and their most recent evaluations. For example, if a class will meet tomorrow and there is one student dropping today and a new student starting tomorrow, it would make sense to run the report as of tomorrow's date.
Select any of the downloadable options at the bottom of the window to create the forms.
NOTE: The Class Evaluation Form is available in HTML and XLS format. If a PDF version is necessary, you can generally use the "Print to PDF" option from your web browser to do so. However, be aware that the resulting PDF may be either incomplete or difficult to read depending on your browser's settings.
